Chronic Care & Remote Patient Monitoring Coordinator

Job Summary

We are seeking a detail-oriented and proactive RPM/CCM Staff Member to support patients enrolled in Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Remote Therapeutic Monitoring (RTM), and Principal Care Management (PCM) programs. This role involves continuous patient engagement, data review, coordination with Providers, and ensuring compliance with program guidelines. The ideal candidate should have strong communication skills, the ability to strictly adhere to checklists and protocols without omission or deviation, and the common sense to resolve patient concerns efficiently.

Key Responsibilities

Remote Patient Monitoring (RPM) & Remote Therapeutic Monitoring (RTM)

  • Monitor and review real-time patient data collected from medical devices such as blood pressure monitors, pulse oximeters, CPAPs, glucose monitors, etc.
  • Identify abnormal readings and escalate concerns to Providers or Respiratory Therapists as needed.
  • Educate patients on the proper use of RPM and RTM devices to ensure accurate data collection.
  • Ensure timely device setup, activation, troubleshooting, and patient compliance tracking.

Chronic Care Management (CCM) & Principal Care Management (PCM)

  • Conduct monthly patient check-ins to assess chronic condition management, medication adherence, and treatment progress.
  • Document all patient interactions, symptoms, and changes in the Electronic Health Record (EHR) system.
  • Assist patients in coordinating specialist visits, diagnostic tests, and follow-up appointments.
  • Educate patients on disease management, medication compliance, and lifestyle modifications.
  • Ensure patients are enrolled in the appropriate CCM, PCM, or RTM programs based on provider recommendations and insurance eligibility.

Administrative & Compliance Duties

  • Work closely with the billing and compliance team to ensure services meet Medicare and insurance requirements.
  • Ensure all orders, referrals, and prior authorizations are completed per provider instructions.
  • Accurately document time spent on care management services to ensure proper billing and reimbursement.
  • Maintain strict compliance with HIPAA, OSHA, and other healthcare regulations.
  • Follow protocols for patient outreach, escalation, and follow-up to improve patient outcomes.

Patient Support & Customer Service

  • Create and educate care plans for patients.
  • Act as a point of contact for enrolled patients, providing guidance and answering questions about their care plans.
  • Address patient concerns with common sense and professionalism, ensuring a smooth experience.
  • Collaborate with the remote call support team to resolve patient queries regarding equipment troubleshooting, appointment requirements, and general health concerns related to chronic conditions. Immediately escalate urgent patient issues to the appropriate Provider or care team.

Technology & Equipment Management

  • Be proficient in EHR systems and remote monitoring software for data entry and patient tracking.
  • Ensure medical devices are functioning properly, troubleshoot issues, and coordinate replacements if needed.
  • Assist in tracking and analyzing patient data trends to provide actionable insights for providers.

Other Responsibilities

  • Perform any additional duties as assigned by management to support remote care programs.
  • Follow chain of command for reporting concerns or operational issues.
  • Willingness to work at both Fayetteville and Sanford locations as scheduled by management.

Qualifications & Skills

  • Experience in RPM, CCM, RTM, or PCM programs is preferred – not mandatory.
  • Background in healthcare, medical assisting or nursing is a plus.
  • Strong attention to detail and ability to follow protocols without deviation.
  • Excellent communication and interpersonal skills.
  • Ability to multi-task and work efficiently in a fast-paced environment.
  • Basic knowledge of chronic conditions such as hypertension, COPD, diabetes, and sleep apnea.
  • Proficiency in medical software, EHR, and remote monitoring platforms.

Schedule & Work Environment

    Medical Assistant

    Job Summary

    We are seeking a highly skilled and detail-oriented Medical Assistant. The ideal candidate must be efficient, compassionate, and capable of handling patient concerns with common sense and professionalism. This role requires strict adherence to established checklists, protocols, and compliance standards to maintain seamless operations.

    Key Responsibilities

    Clinical Support & Patient Care

    • Perform direct and indirect patient care duties under Providers’ supervision.

    • Verify and document patient history, including:

      • Chief complaint, current medications, medical history, allergies, surgical history, family history, social history, and preventive medicine reviews.

    • Measure and record vital signs in the Electronic Health Record (EHR).

    • Triage patients, phone calls, and in-office concerns, escalating when necessary.

    • Explain treatment procedures to patients and prepare both the patient and exam room for examination.

    • Perform spirometry, six-minute walk tests (6MWT), and other diagnostic procedures as per Providers’ orders.

    • Assist Providers with diagnostic tests, medical procedures, and examinations as directed.

    • Hand instruments and equipment to Providers during procedures.

    • Educate patients on treatment plans, medications, and procedures as instructed by the Providers.

    • Ensure patient charts are updated with accurate documentation of assessments, treatments, and follow-ups.

    • Ensure cleanliness and necessary supplies are stocked.

    Administrative Duties

    • Complete accurate documentation in the EHR, including medical services rendered, test results, and supplies used.

    • Schedule follow-up appointments, referrals, and coordinate with specialists as needed.

    • Ensure all orders, prescriptions, and prior authorizations are completed per Providers’ instructions.

    • Check and restock medical supplies and ensure all equipment is functional.

    • Ensure all required paperwork and consents are properly filed in patient charts.

    • Work closely with the remote call support team to address patient queries regarding medication refills, outbound referrals, prior authorizations, and all other clinical queries.

    Appointment scheduling and patient follow-ups

    • Ensure all required tests and follow-ups are scheduled at the check-out.

    • Check and send required outbound referrals as instructed by the Providers.

    • Ensure proper insurance verification, referrals, and authorizations are completed before the patient’s appointment.

    • Coordinate with the billing team to ensure proper documentation for billing accuracy and compliance with healthcare regulations.

    • Assist the front office team as needed with patient check-in/out, referrals, prescription medication management, inbound/outbound calls, and other clerical tasks.

    Customer Service & Problem-Solving

    • Handle patient concerns with common sense and a solutions-oriented approach.

    • Use critical thinking to resolve patient issues smoothly and professionally.

    • Provide a compassionate and empathetic patient experience at all times.

    • Ensure a smooth check-in/check-out process to minimize wait times and improve patient flow.

    Technology & Compliance

    • Be proficient in EHR systems and other required software for fast and accurate data entry.

    • Maintain strict HIPAA compliance and patient confidentiality.

    • Follow OSHA and infection control protocols to ensure a safe clinical environment.

    • Ensure all required paperwork, consents, and pre-visit documentation are in the patient chart before the visit.

    Qualifications & Skills

    • Medical Assistant certification (CMA, RMA, or equivalent), Bio-grads, pre-med / pre-nursing students are preferred.

    • Previous experience in a medical office, pulmonology, or sleep medicine setting is a plus.

    • Strong attention to detail and ability to follow protocols without deviation.

    • Excellent communication and interpersonal skills.

      Physician Assistant (PA) / Nurse Practitioner (NP)

      Job Summary

      We are seeking a dedicated and skilled Physician Assistant (PA) or Nurse Practitioner (NP) to join our Pulmonary, Sleep, Primary & Urgent Care practice. The PA/NP will work closely with our providers to deliver high-quality, patient-centered care in both outpatient and urgent care settings.

      The ideal candidate should have experience in pulmonology, sleep medicine, primary care, or urgent care, possess excellent clinical judgment, and be able to work independently while collaborating with physicians and the healthcare team. This role requires strong diagnostic, treatment planning, and patient education skills to manage both acute and chronic conditions.

      Key Responsibilities

      Patient Care & Clinical Responsibilities

      • Conduct comprehensive patient evaluations, including history-taking, physical exams, and diagnostic assessments.

      • Diagnose, treat, and manage pulmonary, sleep, primary care, and urgent care conditions.

      • Order, interpret, and follow up on diagnostic tests such as pulmonary function tests (PFTs), sleep studies, imaging, and laboratory tests.

      • Develop and implement personalized treatment plans, including prescribing medications, lifestyle modifications, and therapy recommendations.

      • Perform in-office procedures such as spirometry, nasal endoscopy, nebulizer therapy, wound care, and minor urgent care treatments.

      • Provide chronic disease management for conditions like COPD, asthma, sleep apnea, hypertension, diabetes, and obesity.

      • Conduct Annual Wellness Visits (AWVs), chronic care management (CCM), and remote patient monitoring (RPM) reviews.

      • Assist with DME evaluations, including CPAP/BiPAP therapy compliance and patient education.

      • Address acute concerns in the urgent care setting, including respiratory infections, minor injuries, and common illnesses.

      Patient Education & Counseling

      • Educate patients on disease prevention, lifestyle modifications, medication adherence, and sleep hygiene.

      • Provide counseling on CPAP/BiPAP therapy, smoking cessation, and pulmonary rehabilitation.

      • Discuss test results, treatment plans, and follow-up care with patients and their families.

      Collaboration & Coordination

      • Work closely with pulmonologists, sleep specialists, respiratory therapists, and the medical team to provide comprehensive patient care.

      • Collaborate with the remote call support team to address patient inquiries, medication refills, and prior authorizations.

      • Coordinate referrals to specialists, diagnostic testing centers, and home health services as needed.

      Documentation & Compliance

      • Maintain accurate, detailed, and timely documentation in the EHR system.

      • Ensure compliance with Medicare, Medicaid, and private insurance billing requirements.

      • Follow all HIPAA, OSHA, and infection control guidelines to ensure patient and staff safety.

      • Participate in quality improvement initiatives, audits, and accreditation requirements as needed.

      Other Duties

      • Stay up to date with best practices, medical advancements, and evidence-based guidelines.

      • Assist in the training and mentoring of medical assistants and support staff.

      • Attend team meetings, provider discussions, and case reviews as required.

      • Perform other clinical and administrative tasks as assigned by management.

      Qualifications & Skills

      • Valid Physician Assistant (PA) or Nurse Practitioner (NP) license in North Carolina.

      • Board certification as a PA-C (NCCPA) or NP (AANP/ANCC) required.

      • Experience in pulmonology, sleep medicine, primary care, or urgent care preferred (but not required).

      • Proficiency in EHR systems, telemedicine, and remote patient monitoring platforms.

      • Strong diagnostic, problem-solving, and clinical decision-making skills.

      • Excellent communication and bedside manner to provide compassionate patient care.

      • Ability to work independently while collaborating with physicians and the medical team.

      • Familiarity with insurance requirements, billing documentation, and reimbursement guidelines is a plus.

      Schedule & Work Environment

      • Full-time, onsite position.

      • Expected to see a minimum of 15 to 20 patients per day, ensuring efficient and high-quality patient care.

      • Must be willing to work at both Fayetteville and Sanford locations as scheduled by management.

      • May require occasional weekend or evening availability for urgent care coverage or patient follow-ups.
